The researchers employed mixed linear regression models to adjust for sampling variation, former drinker bias, median cohort age, and sex [cite: 41, 42]. The meta-analysis found no statistically significant reduction in the risk of all-cause mortality for occasional drinkers (0 to 1.3 grams per day) or low-volume drinkers (1.3 to 24.0 grams per day) when compared to lifetime nondrinkers [cite: 39, 40].

Upon ingestion, ethanol is oxidized primarily in the liver (and to a lesser extent in the oral cavity and gastrointestinal tract) by the enzyme alcohol dehydrogenase (ADH) into acetaldehyde [cite: 36, 55, 56]. Under normal physiological conditions, acetaldehyde is subsequently metabolized into harmless acetate by the enzyme aldehyde dehydrogenase (ALDH) [cite: 36, 57]. 

However, during active alcohol consumption, acetaldehyde transiently accumulates in tissues, inflicting direct genotoxic damage. Acetaldehyde is a highly reactive electrophile that interacts directly with DNA to form stable adducts, such as N2-ethylidene-2'-deoxyguanosine [cite: 55, 56]. More critically, it forms interstrand cross-links (ICLs), which physically bind the two opposing strands of the DNA double helix together. These ICLs present a formidable barrier to the cellular machinery, actively obstructing DNA replication and RNA transcription [cite: 55, 57, 58].

If cellular DNA repair mechanisms fail to adequately excise and reconstruct these cross-links, the resulting chromosomal instability leads to large genomic alterations, mutagenesis, uncontrolled cell proliferation, and ultimately oncogenesis [cite: 57, 58].

A mutation in the gene coding for ALDH2 limits the body's ability to convert toxic acetaldehyde into benign acetate. For decades, the strongest argument in favor of moderate alcohol consumption was its purported ability to reduce the risk of ischemic heart disease and mortality. Red wine contains negligible, trace concentrations of resveratrol, typically ranging between 0.36 and 1.97 milligrams per liter [cite: 68].
